Most clinics are walk-in with no appointment necessary, however events with sign up required will be marked with an asterisk (*).

What You Should Know

We welcome all patients over the age of 18. 

 

For patients that may need language translation support, clinics typically have medically trained interpreters available on-site.

A list of any current medications is helpful. Individuals do NOT need to bring proof of insurance or identification.

Mobile Health Screenings include blood pressure, cholesterol, A1C, and random blood glucose testing. This includes a simple finger prick with a lancet on the finger to perform all 3 blood tests. Results are given to the patient on site. 

Blood Pressure Screenings will use an arm cuff to determine current blood pressure, and will follow up with a consultation if there are abnormal results.  

 

Your visit may include counseling including health education including heart disease, maintaining good blood pressure, diabetes, and nutrition. Patients will receive documentation with their written results and if they were within normal range or abnormal range.  

 

Screenings may be held at either a mobile health van or onsite, inside a facility such as a church, school, or community center.

 

Appointments may last anywhere from 10-30 minutes depending on complexity and need for follow-up.

Community health workers will discuss test results in depth with the patient and offer recommendations. If the results of any test are abnormal, our clinics follow a specific protocol to help you get the care you need. Depending on the severity of the abnormal results, a patient may be directed to follow up with their primary care provider (PCP). If the patient does not have a PCP, the community health provider will attempt to connect the patient with the appropriate care. 

 

If blood pressure results are abnormal, the community health worker will ask additional questions and offer recommendations. If the patient has high blood pressure and does not have a current PCP, our community health worker will attempt to connect the patient to appropriate care. 

 

Community health workers will follow-up with any high-risk patients we see during clinic.
