Agreements & Forms for Visiting Learners
Medical Students: With the recent recommendations from the Coalition for Physician Accountability, we will begin accepting 4th year elective requests for away rotations on April 1st, for rotations starting after August 1st, based upon program capacity and resources.

Nursing Student and Instructor Onboarding

All onboarding questions should first be addressed with your program's Clinical Coordinator. If questions remain, contact [email protected].
When coming on campus, please park in Garage A on the 6th floor or above.

No further onboarding is required if the instructor is a Luminis Health (LH) employee. Although, if teaching on a specialty unit not previously worked on, a 4-hour shadow day is required before the first day of clinicals. Please email [email protected] to schedule this.
If the instructor is not an LH employee, the program coordinator will provide them with hospital onboarding requirements. A mandatory 4-hour unit shadow day is required before the first day of clinicals. Please email [email protected] to schedule this.
Please do not email the unit educators regarding scheduling; all requests must go through the Clinical Education Dept.
Clinical Group Info Sheet: Instructors are required to complete one at the beginning of each clinical day. This sheet must be provided to the unit Charge Nurse/Clinical Supervisor.
